JOHNSTOWN — COOK – Scott Anthony, 32, Johnstown, passed away June 14, 2012, at Memorial Medical Center. Born Sept. 16, 1979, in Johnstown, son of Samuel J. Cook, and wife, Jean Cook, Tempe Ariz.; and Saundra L. Ream, and fiance, Philip Benson, Johnstown. Preceded in death by grandparents, Thelma Williams, Silas Cook and Patricia Ream; uncle, Brian Cook; aunt, Marie Blair; and cousin, Dolan M. Brosch. Survived by father; mother, and her fiance; stepmothers; siblings, Sonja Lyne Cook, and fiance, Raymond Mickens; and Samuel Jr., and fiance, Mary Rambish, all of Johnstown; and Shawn Cook, Georgia; grandfather, Oran Sr., and wife, Maryann, Windber; special friends, Javin Christian, Renee Simon, Chrysie Stanford and Anthony Woodard; and numerous aunts, uncles and cousins. Member of Heavenly Sent Ministries. Memorial service will be held at 3 p.m. Sunday at Heavenly Sent Ministries, 429 Second St., Conemaugh, the Rev. Donna Williams, officiating. John Henderson Co. Funeral Home is entrusted with the arrangements.
